Referral Process:

• Schedule a phone screening 

• Submit the $45.00 application fee

• Bring identification to first appointment (Driver's License, Social Security Card, State ID, or Passport).

 

Admission Criteria 

• Must be 18 years of age or older.

• Must be a resident of Cuyahoga County.

• Must not test positive for any other drug other than opiates.

• Must not have a current prescription for benzodiazepines.

Transfers from another clinic must fax:

• Authorization of release of Information

• Most recent psychosocial assessment

• Treatment Plan

• Medical documents (exams)

• Urine Drug Screens (last 12 months)

• Medication Dosage (last 12 months)

• List of all medications taken

• Mental health evaluations (if applicable)
